When is it time for hospice?

If you or a loved one has been diagnosed with a terminal illness, there may come a time when the efforts to slow the illness is not working and becomes more harmful than helpful. If that time comes, choosing hospice might be the best choice.

  • A physician has given you a prognosis of six months or less if the illness was to run its normal course.
  • There are no further treatments available, and you want to focus care on comfort and relieving symptoms.

Is HomeReach Hospice a place?

HomeReach Hospice is a home health agency that provides comprehensive healthcare in the patients’ home.


Who is eligible for hospice?

Patients are qualified if:

  • You are eligible for Medicare Part A (Hospital Insurance) or Medicaid
  • Your physician certifies that you are terminally ill and have less than six months to live
  • You sign a statement choosing hospice care instead of routine Medicare covered benefits for your terminal illness*
  • You receive care from a Medicare-approved hospice program
  • Some private pay insurance companies follow different eligibility guidelines and benefits offered can differ from insurer to insurer. Please contact your insurance provider about their hospice eligibility requirements.

Who pays for hospice care?

Hospice care is covered by Medicare, Medi-Cal and most commercial insurance.


Who makes the decision about entering hospice and when?

The decision belongs to the patient. During any point of a life-limiting illness, it is appropriate to discuss all care options, including hospice. Our hospice team is available to discuss options with the patient and their family. The patient/ family member can contact HomeReach Hospice for service in the Los Angeles and Orange County areas. One of the first things the hospice will do, if you decide you want hospice care, is contact your physician to make sure he/she agrees that hospice care is appropriate. Your doctor must certify that you are terminally ill and may have less than six months to live, if the disease takes its expected course. Even if you live longer than six months, you can get hospice if your doctor continues to certify that you are terminally ill.
