HomeReach Hospice believes in bringing care to you, wherever you call home and strives to provide an environment of care that is comfortable for our patients and their families. Often, this means providing care in the patient’s own home, and our team of professionals’ schedule regular home visits to ensure that medical, social, and emotional needs of our patient and family are met.

Our care team may include the following:

Hospice Medical Directors
  • Responsible for developing a comprehensive care plan with the team
  • Ensures that protocol is met for pain and symptom management and oversees entire team
  • Your nurse, together with your PCP and medical director work together to help control your symptoms and increase your comfort
Primary Care Physicians
  • Specialize in palliative care
  • Provide home visits as needed
  • Work closely with the hospice medical director and registered nurse to develop an individualized care plan
  • Ensure medical needs are met
Registered Nurses
  • Responsible for your overall care
  • Coordinate, supervise and implement the plan of care
  • Provides support and valuable information to family/ caregiver on how to best care for you
Spiritual Counselors
  • Provide counseling, guidance and education to your loved one and family members
  • Calls to visit and listen, comfort and emotional support when needed
Home Health Aides
  • Assist with personal care, meals, light housekeeping
  • Help with activities and other care activities to supplement what families and caregivers are able to provide
Social Workers
  • Assist with community resources, and provide emotional support regarding terminal illness for patient and family members
  • Provide support in dealing with non-medical issue
  • Accessing needed resources for future planning
Hospice Volunteers
  • Provide companionship for the patient or family
  • Reading, talking, playing cards or board games
  • Extra level of companionship

HomeReach support services may include:

  • Nursing care available on a 24-hour basis
  • Durable medical equipment, medical supplies and medications related to hospice care
